Undergoing Primary Tip Open Rhinoplasty in San Diego, CA
This is another example of primary tip open rhinoplasty in a female patient who was from San Diego, CA. This young lady was unhappy with how her nasal tip appeared droopy (ptotic) and was excessively wide. The nose was also asymmetrical (crooked) in terms of the front view. During her consultation with me, we performed digital imaging of her nose using a plastic surgery software program that allows us to visualize what certain changes might look like on her face. She decided to proceed with the surgery, which was done in our own private ambulatory surgery center. I performed an open rhinoplasty to allow optimal exposure and visualization of her nasal tip region. I then lifted her nasal tip and brought it to a more midline position. In the process, I was also able to give her nasal tip a more narrow appearance without looking pinched. I think the final result shows a natural looking nose that is very fitting for her face. In fact, the patient and her family were so happy with the rhinoplasty results that another member of their family just had the same thing done by me!
Female Middle Eastern Cosmetic Nose Surgery
This is a female Middle Eastern rhinoplasty patient who sought out an expert rhinoplasty surgeon to reshape her nose. Her main issues involve the nasal tip and columella region. More specifically, this particular patient had what is called a hanging columella. This means that the area in between the nostrils hung down too low – giving the appearance that it was drooping off of the nasal tip region. In addition, her nasal tip was poorly positioned. Furthermore, her nasal bridge had a bump, or hump, deformity that she disliked. I ended up performing a rhinoplasty procedure to reshape her nose. Careful thought and planning was required above and beyond your average rhinoplasty surgery. This is because I was dealing with an ethnic rhinoplasty patient who has a Middle Eastern heritage. In these situations, plastic surgeons must be even more careful to create a final result that is natural appearing and balanced with the face. As you can see in the following before and after ethnic rhinoplasty photos, her cosmetic surgery result reveals a nose that is in harmony with the face – and one that looks like it belongs on an Middle Eastern ethnic face.

One Year Anniversary Rhinoplasty Results

Dr. John Hilinski recently took some time to answer questions about what patients can expect when they visit our San Diego plastic surgery practice for a rhinoplasty consultation. Below are Dr. Hilinski’s answers about his own consultation process and why he encourages his prospective patients to seek out a second opinion if they feel unsure about the procedure.
Question: What does a rhinoplasty consultation entail?
Dr. Hilinski: The consultation entails a thorough dialogue, focusing on the unique ways each patient wishes to shape the nose. I always like to hear the patient’s concerns framed in his or her own words. The consultation also includes a detailed examination of the face as a whole and the nose in particular. It is critical to analyze the nose in context of the face since achieving harmony with the surrounding facial features is the ultimate goal of rhinoplasty.
In addition to assessment of the skin and framework of the nose, it is vital that the internal workings of the nose be evaluated to ensure proper function. Computer morphing technology is a very powerful tool for rhinoplasty surgeons that allows us to use actual photographic images of the patient’s nose to visually demonstrate what the nose might look like after surgery. This process provides valuable insight into the results the patient is truly envisioning, which sometimes differs from what they describe using words alone. I can also determine whether or not the patient is being realistic in terms of what rhinoplasty can really do for them.
Question: How long is the consultation and how many office visits are required?
DH: My office typically reserves upwards of one hour for new rhinoplasty consultations. This provides us with ample time to discuss all of the relevant issues with the patient’s nose. A good percentage of our rhinoplasty patients feel sufficiently comfortable after this one visit to make an educated decision about having the surgery. If patients decide to proceed with the rhinoplasty procedure, they will return to the office again for a formal pre-operative visit. During this second visit, we prepare the patient for the actual date of surgery by reviewing pre-operative instructions and guidelines.
Question: Do you recommend family members or friends attend the consultation?
DH: In some cases it is helpful to have a family member or friend accompany the patient during the rhinoplasty consultation. Family members or friends who are close to the patient will be able to provide insightful comments, especially when it comes to viewing the morphed computer images. We also email the morphed images to the patient for further review, which is very helpful in those instances where family and friends are unable to attend the consultation. Ultimately though, I encourage my rhinoplasty patients to try and make the final decision on their own. After all, rhinoplasty is a very intimate decision that should be largely influenced by personal motivation.
Question: What is the next step?
DH: The next step is contemplation. I think it is very healthy when patients go home and let the recommended changes to their nose truly sink in. In some cases, patients seek out different opinions from other rhinoplasty surgeons, which I encourage. I feel quite confident in my abilities as an expert rhinoplasty surgeon, and in a good majority of cases, patients who seek out other opinions will be even more assured about choosing me as their rhinoplasty surgeon.

How Can You Best Prepare For Rhinoplasty? Dr. John Hilinski Answers Your Questions
Part one of our two-part rhinoplasty series entitled “San Diego Plastic Surgeon Answers Questions About Rhinoplasty Consultations” detailed what Dr. John Hilinski’s nose surgery patients can expect during the initial consultation process. In this post, our experienced plastic surgeon will outline how to prepare for rhinoplasty surgery, what to expect the day of surgery, and what he expects from patients during the recovery period.
Question: What can a patient do to make sure they are prepared for surgery?
Dr. Hilinski: Given how frequently I perform rhinoplasty, my office has developed a fairly comprehensive guide we give patients to prepare for the surgery. Much of this preparation involves avoiding certain types of medication that might encourage bleeding and bruising. Proper preparation also includes educating the patient regarding recommended methods to clean the areas around the nose following the procedure.
Question: What does the day of surgery entail?
DH: On the day of surgery, the patient checks in to our accredited ambulatory surgery center and meets the staff, including our board certified anesthesiologist, registered nursing staff, and surgical technician. The patient then completes any necessary clerical items still outstanding before entering the operating room. Once the patient is gently put to sleep by the anesthesiologist, the rhinoplasty surgery begins. A typical primary rhinoplasty surgery (for patients with no prior rhinoplasty) takes about two to three hours to complete. Within an hour, the patient is able to return home in the company of a friend or family member.
Question: Are there important recovery instructions for the day of the surgery?
DH: We have a fully developed guide that includes detailed instructions regarding the early healing process following rhinoplasty surgery. Our registered nursing staff will review these instructions prior to departure from the surgery center to ensure that each patient has clear expectations for the initial recovery period.
Question: Are there important recovery instructions for the week after surgery?
DH: Even one week out from surgery there are important instructions to follow. For example, I usually will remove bandages from the nose around five to six days out from rhinoplasty surgery. Although most patients begin to feel more comfortable out in the public eye around this time, there are still many restrictions that need to be minded. This includes avoiding even minor pressure to the nose, such as rolling over onto the nose inadvertently during sleep.
Question: How long until patients typically say they feel like themselves again?
DH: Most patients start to feel comfortable resuming their normal daily routine after seven to ten days. At this point in the recovery process, the bandages and sutures have been removed, so there are no overt signs of surgical recovery other than some swelling. Even bruising will be essentially resolved. Borrowing a term from one of my mentors, I like to tell patients that at this point they will look ‘supermarket presentable’—standing in line at the local supermarket, it will not be apparent that they have just had surgery. I don’t recommend that patients resume their exercise routines for about two to three weeks following rhinoplasty. In general, most patients will see 70% of the swelling resolved in about three months. There is still a moderate amount of healing that will occur over the course of that first year.
Question: Is there anything else a patient should know about the recovery process?
DH: I think the most important factor to consider when recovering from rhinoplasty is the length of time involved. Many patients do not grasp just how long it takes to see the final results of the rhinoplasty surgery. This is why I try to emphasize this point to all of my patients who are considering the surgery. Rhinoplasty is not the type of cosmetic surgery that will show the final results within five to six months. Even 10-12 months after rhinoplasty, there are changes that continue to manifest.
In addition, rhinoplasty recovery is very much a collaborative process. I certainly have my responsibilities as a rhinoplasty surgeon on the day of the surgery, but the rhinoplasty patient is also responsible for part of the healing process. Patient responsibilities include keeping their incisions clean, avoiding trauma to the nose that would compromise the results, attending all of the recommended postoperative office visits, and performing nasal exercises such as taping the nose when indicated during the postoperative recovery. These are very important aspects of the rhinoplasty recovery process that cannot be done by the rhinoplasty surgeon – they are the full responsibility of the rhinoplasty patient. I make sure that all of my rhinoplasty patients feel comfortable with this type of collaborative process well before we begin surgery.

More specifically, she disliked the bump on the bridge and the fact her tip hung down too low. This is actually a very common complaint among rhinoplasty candidates. I ended up performing her cosmetic rhinoplasty using an open approach technique. I reshaped the bridge by making it appear straighter. I then brought her tip up to a more appealing position, but avoided making the tip too high. As you can see from her before and after photos, there is an overall marked improvement in the appearance of her nose.
